Here is the procedure.

1.      Click “Submit file” on top.

2.      Select “Upload file From Your Computer”

3.      Browse to file on local machine

4.      Click “Upload”

5.      Under Description:

a.      Create nice document title

b.      Select Category to decide which page on your state you want document to be on.

c.      Write a good document description

d.      Click Save

2012 ISA INSURANCE PROGRAM

Team insurance covers your team for any amateur play or practice.

Tournament coverage can be purchased with a credit card. Application can be emailed, called in or faxed – 1-804-732-2968. Certificates can be issued on Friday prior to the beginning of your tournament. Just make sure that you purchase prior to the start of the tournament. Tournament insurance covers your tournament for liability and medical accident for teams. The cost is $7.00 per team for youth and $16 per team for adult. Detail information page attached.

ISA registered umpires are covered for any ISA sanctioned activity. Coverage includes $2,000,000 general liability policy and secondary Accident Medical of $50,000 with a $100 deductible.

Team insurance for ISA teams is available for purchase on the ISA website. Team insurance includes $2,000,000 general liability and $100,000(youth), $50,000(adult) limit medical accident with a $100 or $250 deductible for youth and $500 deductible for adult. This insurance covers your team for any amateur play or practice. Detail information page attached.
